All seniors are required to fill out the Diploma Name Form ON THE COUNSELING WEBSITE.  


You must use your legal name for this document.  The name you submit on the Diploma Name Form will be how it appears on your official diploma.  The district has implemented a new policy concerning changes.   A $30 fee will be charged for any name changes submitted after January 26, 2015.
