SENIOR SURVEY
REQUIRED FOR GRADUATION
Lesson Objective: To
complete Senior Exit Survey and Final Transcript Google Document
Procedure:
A. Announce to Advisory:
Today we will be filling out
two required documents to inform the counseling office what you plan to do
after graduation and where to send your final transcript.
A final transcript is a record
of your entire high school career. This
document will have your final GPA and Class Rank. Transcripts will be mailed approximately 2
weeks after grades are due.
You will need to complete two
items to complete the process;
1. The Grad
Survey on Family Connection
2. The
Final Transcript Request Google document
The information may seem
repetitive; however, the students must complete both documents in order for the
transcript request to be successful.
Your college admission process will not be complete without these
documents. Colleges require a final
transcript.
B. Process:
Students
go to LHS website. Under Student Resources, click on Guidance. Click on Family Connections Website
on the left side of the page.
Students
click on the About Me tab after they have logged into their
accounts. Click on Grad Survey1
on the top left side of the page.
1. Complete
your Last, First and Middle Name in the appropriate boxes.
2. Complete
the next section “On September 15, 2015, I will be ….” You may use the text box for an option not
listed.
3. Indicate
your area of study. You may type
undecided.
4. Enter
information ONLY if you are going into the workforce.
5. The State of Missouri requires high schools
to conduct a 6 month follow-up study on recent graduates. Please provide a cell phone number where you
can be reached in December 2015. Include
your area code and cell number in the appropriate box. If you do not have a cell phone number, enter
NONE.
6. Provide a NON-SCHOOL email address where you
can be reached. Do not include your
Liberty email address. If you do not
have an email address, enter NONE.
7. This
is the explanation for 8-14. Students do
not answer this question.
8-14: Student input is helpful as we develop and
grow our programing. We appreciate your time!
15 and
16: Your suggestions are important to us.
It is
important to note the following:
You
will need to request your Final Transcript on the Senior Final Transcript
Form Google document. The Google
document is located on the front page of the Guidance Website.

Books for Africa!
What:
A book drive for multiple libraries that are being built in the Horn of Africa and Sudan. Many, many books are needed for these, and the church needs all they can get.
When:
May 7 and 8
Why:
These libraries are being built for children who are escaping prosecution in these countries. WE ARE FOCUSING ON CHILDREN’S BOOKS! All books are welcome, but the focus is on basic level reading and small children’s books.
Where:
Take the books you would like to donate to your Advisory classroom.
Please, do not try and send damaged books. The books have to be in presentable condition to be sent to Africa. Jump ropes and frisbees are accepted as well.
