SENIOR SURVEY
REQUIRED FOR GRADUATION

Lesson Objective: To complete Senior Exit Survey and Final Transcript Google Document
Procedure:
A.    Announce to Advisory:
Today we will be filling out two required documents to inform the counseling office what you plan to do after graduation and where to send your final transcript.
A final transcript is a record of your entire high school career.  This document will have your final GPA and Class Rank.  Transcripts will be mailed approximately 2 weeks after grades are due.
You will need to complete two items to complete the process;
1.    The Grad Survey on Family Connection
2.    The Final Transcript Request Google document
The information may seem repetitive; however, the students must complete both documents in order for the transcript request to be successful.   Your college admission process will not be complete without these documents.  Colleges require a final transcript.

B.    Process:

Students go to LHS website. Under Student Resources, click on Guidance.   Click on Family Connections Website on the left side of the page.

Students click on the About Me tab after they have logged into their accounts.  Click on Grad Survey1 on the top left side of the page.

1.    Complete your Last, First and Middle Name in the appropriate boxes.
2.    Complete the next section “On September 15, 2015, I will be ….”  You may use the text box for an option not listed.
3.    Indicate your area of study.  You may type undecided.
4.    Enter information ONLY if you are going into the workforce.
5.  The State of Missouri requires high schools to conduct a 6 month follow-up study on recent graduates.  Please provide a cell phone number where you can be reached in December 2015.  Include your area code and cell number in the appropriate box.  If you do not have a cell phone number, enter NONE.
6.  Provide a NON-SCHOOL email address where you can be reached.  Do not include your Liberty email address.  If you do not have an email address, enter NONE.
7. This is the explanation for 8-14.  Students do not answer this question.
8-14:  Student input is helpful as we develop and grow our programing.  We appreciate your time!
15 and 16: Your suggestions are important to us.

It is important to note the following:
You will need to request your Final Transcript on the Senior Final Transcript Form Google document.  The Google document is located on the front page of the Guidance Website.

The Leadership Class has prepared a fantastic fundraising opportunity for Special Olympics with the implementation of the Strut-n-Stroll event to be hosted on April 24th.  This event will take place on our track and your class and/or individual participation would be welcomed and appreciated.

The main component of the fundraising is within the Advisory $25 Challenge, however, there will be items for purchase and prizes to win if you go to the track to participate.   You can take your class for the entire hour, for 30 minutes, or even for just one jaunt around the track.  We want to show our support of this fantastic student driven project, if it works within your teaching schedule.

Enjoy!

#LHSBeTheChange Advisory:
Acts of Kindness Project

City Mission Clothing Drive

Klein's Senior Advisory kick started a clothing drive for City Union Mission (date extended to March 26th).  If you want to support this cause please donate clothes to any LHS advisory.  Contact Angie Klein or Charity Stephens for more information.

Tag Your It (Tagging Kindness at LPS)

Stephens' Senior Advisory is kick starting the Tag Your It Project. Your Advisory teacher may get "tagged" by receiving an email with the image above.  Your advisory class must then decide if you want to take the challenge to spread kindness via LPS.  All you have to do is create posters of kindness to delivery to an LPS school below.

Tag Your It Directions:  Once your class accepts the challenge (You do not have to wait to be tagged! Just jump on in and choose a school to make posters for! Just email Charity Stephens to tell her what school you chose:)
  1. Choose one of the schools below to make kindness posters for
    1. Manor Hill
    2. Lillian Schumacher
    3. Ridgeview
    4. EPiC
    5. Alexander Doniphan
    6. Shoal Creek
    7. Liberty Oaks
    8. Discovery Middle School
    9. Liberty Middle School
  2. Forward the "tag" email to a new advisory teacher from your grade level (also Cc Charity Stephens) and include the school you will make posters for so that each time the email is forwarded the new advisory will see all the schools that have been chosen. It is ok to double up on schools!!! Let's just try to share the kindness with ALL of these buildings!
  3. How will these be delivered? Great question! Have your advisory decide:
    1. Maybe they have a parent that works at one of these schools
    2. Maybe a group of students would like to contact the building principal for permission to hang the posters after school
    3. Maybe you can contact the building principal or counseling staff and ask to send them by pony...maybe that school has a group of kids that can hang them
    4. Maybe it's just easiest to give the posters to Charity Stephens and she will figure it out
    5. Maybe there is a creative delivery idea not listed here...just take action & spread kindness #LHSBeTheChange
Serteens Spare Change Challenge

Today's the start (March 23) of the Serteens Spare Change Challenge to raise money for the Victory Project, a charitable organization benefiting KC children with illness supported by Sporting KC. To help motivate your Advisory class to give, the Serteens club is sponsoring some rewards
  • Each class raising at least $100 over the week will receive donuts.
  • The class collecting the most $$ for the week will receive a pizza party.
A Serteens club member will stop by during Advisory each day to collect your change.  He/she will have a pass from me, so you'll know that person is a Serteen!! The Serteens will count the change each day after school, and I'll send out the totals. The list of classes participating is below.  Let Sara Langford know if you haven't signed up yet and your advisory class or area (office, guidance, LMC, etc…) would like to participate!  If I missed your class in the list, please let me know.
Thank you for your help!!
